Facility Portal Privacy Notice

This is a draft placeholder for the ClinicGenZ facility portal. It must be reviewed before production use.

Information we may collect

Facility application data may include facility name, category, business registration details, address, GPS latitude/longitude/accuracy/timestamp, public contact details, services offered, authorized representative details, referral code, declarations and consent records.

Purpose

Information is collected to review facility applications, contact representatives, verify eligibility for pilot listing, prepare app listing data, manage admin review and support future portal functions.

Referral code privacy

The referral code may be stored with the application. Private facility owner details, documents and internal review notes must not be shown to the referrer.

Public listing data

Only approved public facility information should be shown in the ClinicGenZ app after admin verification. Private application information, GPS precision details and uploaded documents must remain restricted.

Documents

Document upload should only be enabled after secure storage, access controls, file restrictions and admin audit rules are implemented.
